This form is set up to collect data for registration. Once you start the application, your information is automatically stored for SMWA office use even if you do not complete form or make a purchase.  To ensure your form is properly reviewed by the administration, please complete the form fully, enter the payment option and then sign the form.  The form will be reviewed to make sure it was completed correctly then a PayPal invoice will be sent to the payment email provided.   if you need help completing the registration, please click HERE.

*FEMA Courses are not charged in the 2024 SMWA registration.
